Accessible Parking Permits

Please have your healthcare provider complete the Application for NYS Accessible Parking Permit (MV-644.1). The application is also available at your doctor's office and on the DMV website. Please be sure to include your Driver's License Number in Part 3 of the form.

If your tag was lost or stolen, please complete the Lost or Stolen NYS Accessible Parking Permit Affidavit and return to the Town Clerk's Office.

Permanent Disability (Blue Tag)

Permanent tags are valid for two years at a time. You will need a completed Application for NYS Accessible Parking Permit (MV-644.1) the first time you get a permanent tag. After that, please bring in your expired tag and we will replace it.

Temporary Disability (Red Tag)

Temporary tags are valid for a maximum of six months. To renew, you will need a new Application for NYS Accessible Parking Permit (MV-644.1) signed by your healthcare provider.
